New patients - please arrive on time, during the initial visit there is evaluation, as well as, mandatory arbitration consent paperwork to be filled out. Evaluation of your condition is assessed and a treatment plan is requested from your Insurance provider. A follow-up visit(s) is mandatory, the number of which depends on your progress.
After the first few sessions, we are able to determine the lesser patient responsibility amount for each follow-up session.
I will re-imburse you as soon as I receive the (EOB) Explanation of Benefits and the check from your health insurance company; Aetna takes two weeks to process, Cigna three weeks, United four weeks to send out their checks to the provider. United Healthcare Shared Services (UHSS) takes five weeks to process.

We start with a medical office visit (I am an out-of-network provider) as a new patient with intake to discuss your medical history. This includes palpation and visual assessment, filling out forms and verbal discussion. There are legal arbitration consent forms, a group insurance policy form, a patient's right to privacy form and an intake questionnaire.
The treatment session is tailored to address the issues of priority listed and discussed during intake.
First appointments are around 75- 85 minutes Once your out-of-network deductible has been met, I will be paid directly by your health insurance PPO plan 60-80% and your co-insurance payment is 20-40% depending on your policy contract.
